By using the Arrive Health website www.arrivehealth.com (the “Site”), you agree to the terms of this Privacy Policy and Notice (PPN), as outlined below. If you do not agree to these terms, please do not access or use this site.
This PPN describes how Arrive Health (“we,” “us” or “our”) collects and uses personal information about you when you visit our Site as well as your choices about how personal information will be used. By using this Site, you accept the privacy practices contained in this PPN. We encourage you to read this PPN in full to understand our privacy practices before using this Site or submitting any personal information.
Unless otherwise agreed in a contract signed by an authorized representative of Arrive Health, and except to the extent a state privacy regulation applies, the validity, construction, and effect of this PPN will be governed by the laws of Delaware, without giving effect to any state’s conflict of laws principles. Any dispute will be resolved in a forum selected by Arrive Health.
SCOPE
The Arrive Health PPN applies only to information collected from visitors to www.arrivehealth.com. It does not apply to information collected from users of our services, which is instead governed by the applicable agreements with said users or their employers. In addition, Arrive Health affiliates, subsidiaries, business partners, ad networks, linked sites, and other similar third parties may have their own subdomains and websites with their own websites and privacy practices. We encourage you to read the privacy notices of all websites you visit to understand their privacy practices and your options.
PERSONAL INFORMATION WE COLLECT

Rather, website forms made available to users of the Site are intended for business communications. We may use the information collected for the following purposes:

USE IN THE UNITED STATES
Arrive Health operates and our Site is maintained in the United States. Regardless of where you access the Site, the information collected as part of that use will be transferred to and maintained on systems located in the United States. By submitting information to us, you agree to the transfer, storage, and processing of that information in the United Sates, as well as the application of U.S. laws and regulations.
RIGHTS UNDER STATE LAW
Certain Site users may have additional rights under their state of residence, including if you are a resident of California, Colorado, Connecticut, Nebraska, Nevada, Utah, Virginia, or other states that adopt applicable data privacy regulations. - Request that we provide you with information about whether and to whom we have disclosed or sold your information, as well as request that Arrive Health not sell your information (Note: Arrive Health does not sell personal information collected on the Site)

- Have your information deleted (subject to exceptions described in the applicable state regulations)
- Not be discriminated against because you have exercised any of these rights.
You may make any of the above requests by contacting Arrive Health through one of the two following methods: email privacy@arrivehealth.com or submit your request to our corporate address below. You may make such a request no more than twice in any twelve (12) month period. Arrive Health reserves the right to request information to verify your identity in order to respond to your request, and we will respond within forty-five (45) days.
OPTING OUT OF TEXT MESSAGES
If you have received a text message from Arrive Health and wish to stop receiving such communications, please follow the prompts provided within that message. If you are unable to find our SMS number, please text ‘STOP’ to 720-730-9667. You may also opt out of receiving text messages from Arrive Health by sending your first name, last name, and phone number to privacy@arrivehealth.com. Please note that if we have sent a text message on behalf of your healthcare provider, this opt-out will only apply to messages sent by Arrive Health.
UPDATING OR DELETING INFORMATION PREVIOUSLY SUBMITTED VIA THE SITE
If you wish for information that you previously submitted via the Site to be updated or deleted, please send an email to contact@arrivehealth.com. We will use reasonable efforts to update your information as soon as possible, including in accordance with applicable state regulations.
HOW WE PROTECT YOUR INFORMATION
We use reasonable and appropriate physical, technical, and administrative industry safeguards to protect information from unauthorized access, use, loss, misuse, or unauthorized alteration of information shared on this Site. We will make reasonable efforts to protect the information submitted via the Site from unauthorized access, using commercially reasonable security products (e.g., firewalls), as well as carefully developed security procedures and practices. Notwithstanding our security safeguards, it is impossible to guarantee 100% security in all circumstances.
